GALAXY KIDZ EDUTAINMENT CENTER SURVEY

1.  This survey is organized by New Galaxy Entertainment 2006 Company Limited (“Galaxy”).

2.  To continuously improve the quality of the Edutainment Center, Galaxy Kidz Edutainment Center will distribute a questionnaire to eligible participants. The following eligible guests will be able to complete the questionnaire.

• Must have a confirmed registration and physically attended the activity.
• Must scan the dedicated QR code present at the venue. 

3.  The activities organized by the Galaxy Kidz Edutainment Center include “Wavey’s Studio”, “Wavey’s Lab”, and “Wavey’s Kitchen”.

4.  By participating in this Survey, you may opt to provide the relevant personal data (including without limitation, your name, contact number, email address and GEGPC number) (“Personal Data”) to Galaxy and any company of the Galaxy Entertainment Group (the “Galaxy Group”). You hereby fully agree and understand to the Terms and Conditions of this Survey, and authorize Galaxy Group to collect, use, store and process automatically or mechanically any Personal Data provided by you, for any marketing and promotional purposes in respect of our news, promotions and other services such as F&B, hotels & accommodations, retail, cultural and entertainment services, and to customize our marketing offers and to facilitate our market research & surveys, as well as for direct marketing purposes. In addition, you also expressly authorize us to share, disclose and transfer your Personal Data to any of our employees, affiliated companies with Galaxy Group, agents, contractors and third-party service providers for the above said purposes.
