Dining Experience Survey

1.  This survey is organized by New Galaxy Entertainment 2006 Company Limited ("Galaxy").
2.  Any participant who dined in our participating restaurants at Galaxy Macau, scanned the QR code provided by our Team Members on dining receipt is eligible to fill in our dining experience survey (“Survey”).
3.  Participating restaurants at Galaxy Macau include

• StarWorld Hotel: Feng Wei Ju, Temptations, Sensations, Whisky Bar, Lobby Lounge, Passion. By Gérard Dubois, Chang Sheng Fang.
• Galaxy Macau: Bei Shan Lou, Blossom Palaces, Butao Ramen, CAFÉ DE PARIS MONTE-CARLO, Casa da Rosa, CHA BEI, gaga, Kyo Watami, Lugang Café, Pak Loh Chiu Chow Restaurant, Pang’s Kitchen, Tastes of Asia (all food stalls except HEYTEA, GODIVA and Häagen-Dazs™), The Apron Oyster Bar & Grill, Galaxy Macau Tam Chai Yu Chun, Yoshimori Teppanyaki, Passion. By Gérard Dubois, The Noodle Kitchen, Yamazato, Nagomi, Saffron, Spicy Sichuan, Andaz Bar, Andaz Kitchen, Raffles Lounge & Terrace, Long Bar, Putien, Pony & Plume, Waso Café, Breeze Café (opened in Summer only), Cabana Bar (opened in Summer only).

4.  By participating in this Survey, you may opt to provide the relevant personal data (including without limitation, your name, contact number, email address and GEGPC number) (“Personal Data”) to Galaxy and any company of the Galaxy Entertainment Group (the “Galaxy Group”). You hereby fully agree and understand to the Terms and Conditions of this Survey, and authorize Galaxy Group to collect, use, store and process automatically or mechanically any Personal Data provided by you, for any marketing and promotional purposes in respect of our news, promotions and other services such as F&B, hotels & accommodations, retail, cultural and entertainment services, and to customize our marketing offers and to facilitate our market research & surveys, as well as for direct marketing purposes. In addition, you also expressly authorize us to share, disclose and transfer your Personal Data to any of our employees, affiliated companies with Galaxy Group, agents, contractors and third-party service providers for the above said purposes.
